In order to vote while attending a Full Board or Committee meeting, a CB2 Member must have one of the following forms on file with the District Office.  Members my click on the links below to download a Word version of the forms.

Please note that, under the Open Meetings Law, a Board Member’s participation in a meeting counts toward the quorum for that meeting only if that Member is either (a) present in person at the meeting venue, or (b) has a certificate of temporary or permanent disability on file with the Office.  Filing an Extraordinary Circumstances Form allows a remote participant to vote only if a quorum is otherwise present.
