Community Health Assessment

Mission Statement

The West Side Community Health Needs Assessment is a systematic method of identifying the health needs of the residents of the West Side of Manhattan.  By studying the demographics of our community, the health status of local residents, current health care resources and utilization patterns, access to care, and service needs and gaps, our goal is to identify opportunities to improve the delivery system to better meet the needs of our community.

North Shore-LIJ Health System in partnership with the Hunter School of Urban Public Health will lead a process that gathers and analyzes relevant data.  North Shore-LIJ is committed to working closely with our Steering Committee, which will be comprised of local elected officials, community boards, health care providers and advocates, and community-based organizations.  Within the framework of an evidence-based understanding of the health care needs of our community, the Steering Committee will be able to make recommendations on improving access to health care now and in the future.
